Requesting Records – Elementary School

Student Records

Parents and guardians or School Administrators may request electronically or in person a copy of their child’s school records. Within 10 business days of submitting a request to the school, a parent/guardian will receive a copy of the following student records:

  • Academic records
  • Discipline records
  • Results of screenings and diagnostic test
  • Individual Education Plan (IEP)
  • Individual Accommodation Plan (IAP)
  • Other student-specific file or document maintained by the school

To request records in person, please make an appointment prior to visiting the Jefferson Avenue Campus (located at 1116 Jefferson Avenue, New Orleans, LA 70115). To request electronically, please complete the form below.
